Catholic Charities of Long Island is a trusted partner in the HEALI Social Care Network for Long Island, helping eligible Medicaid members connect with services that support health and well-being.

Through HEALI, Medicaid members can complete a simple screening for Health-Related Social Needs and be connected to programs that may help with needs such as food, housing, transportation, utilities, and other support services.

Eligible Medicaid Managed Care members may qualify for additional services at no cost.
